Use this simple CSV as a starting point for recording the task, its steps, hazards, controls and review. It is intentionally blank: your team supplies the job-specific information and decides what else the document needs.
CSV file. Open in a spreadsheet application and add or adapt columns to suit your process.
Start with the job and location, then describe the work in the order it will happen. Keep each step specific enough that someone familiar with the task can recognise what is being done.
Break the activity into a clear sequence. Include preparation, access, the work itself and pack-up where relevant.
Record what could cause harm at that step, including equipment, energy sources, environment, people and interaction with other work.
Describe the measures that prevent exposure or reduce risk. Name the practical action, not just a broad label such as “be careful”.
Identify who will put controls in place and who checks the document with the crew before work begins. Add date, site and sign-off fields as your process requires.
This educational example shows how to turn a broad job description into steps and questions for the crew. The hazards are not exhaustive. Your team must assess the real task, choose and document controls, assign responsibility and arrange review before use.
| Job step | Potential hazards to assess | Questions before selecting controls |
|---|---|---|
| Confirm the delivery and work area | Vehicle movements, pedestrians and restricted access | Where will the delivery take place? Who will confirm access and separation arrangements? |
| Move materials to the agreed location | Load weight, handling needs and obstructions | What handling method and equipment suit the actual load? Who will check the route and conditions? |
| Finish the delivery and check the area | Packaging, remaining obstructions and changed access | Who will confirm the area is clear and communicate any remaining hazards before other work resumes? |
Revisit the analysis if the equipment, route, materials or conditions change. Confirm your employer's process and the applicable provincial, territorial or federal requirements.
